[BCP] column generation broken in 1.2

Hi all,

I'm sad to say that I just realized that column generation is broken 
in the 1.2 series... :-( (dual objective limit is set in the LP solver 
when it should not be set). I'm thinking on the best way to fix it, 
but it's non-trivial. Still, I hope to have a solution within a week.

I'm going to update the examples as well, so that they compile and run 
with the 1.2 series.
