[Ipopt] Restoration Phase Failed

Hello,
Why the ipopt always stop with "Restoration Failed!" when I add a new 
constraint into the problem.
It works well without this constraint and the solution even satisfies 
the new constraint.
Is there anyone know the reason for this? Thank you.
