[Couenne] large number of error messages

And another issue with the output: running Couenne on the CUTE test set
<http://orfe.princeton.edu/~rvdb/ampl/nlmodels/cute/index.html> produced
~1GB of identical error messages:

  Error in an AMPL evaluation. Run with "halt_on_ampl_error yes" to see
details.

It would be nice if the error message was only produced once per
optimization problem to avoid flooding the output by domain errors and such.
