[BCP] release 1.2.2 created

Hi All,

I have created a new bcp release: 1.2.2. This should be a fairly 
robust release.

Finally all examples and applications compile and run correctly (I 
think :-).

I have chased down a nasty and rarely occurring bug in column 
generation schemes.

Let me know if you find any problems.
