[Dip] Release 0.9.12

[Ted Ralphs]

I have now fixed all the stand-alone examples in stable 0.9 and made a new
release 0.9.12. Examples should be working also in trunk now and we are
hard at work on stable 0.91, which should have its first release very soon.

The apps should all work in trunk now as well and trunk should be safe to
use with one exception. There is still a problem generating CGL cuts when
using "master only" variables (variables that are not present in the
linking constraints that are relaxed).
